Osha defines a ladder safety system as a system designed to eliminate or reduce the possibility of falling from a ladder.

When a ladder safety system is provided the maximum clearance between side rails of the extension must not exceed 36 inches 91 cm. Ladders must be free of any slippery material on the rungs steps or feet. Do not use the top step rung of a ladder as a step rung unless it was designed for that purpose. Cages and wells are not ladder safety systems.

A ladder safety system usually consists of a carrier safety sleeve lanyard connectors and body harness. Falls from ladders are a leading cause of fatal injuries in general industry and construction. It includes details like the weight loads the allowable space between rungs and when cages are required. Do not use a self supporting ladder e g step ladder as a single ladder or in a partially closed position.

Ladders newly installed or altered after november 19 2018 require a ladder safety system or personal fall arrest system. For existing fixed or permanently mounted ladders that extend 24 feet above a lower level osha developed a timeline to phase out the use of ladder cages as a means of fall protection. The new rule phases in a requirement for employers to provide ladder safety or personal fall arrest systems for fixed ladders that extend more. For through ladders the steps or rungs are omitted from the extensions and the side rails are flared to provide not less than 24 inches 61cm and not more than 30 inches 76 cm of clearance.
